EOE POSITION The case manager is responsible for assessing patient needs by optimizing utilization of hospital resources by ensuring patients are directed to the appropriate level of care and meeting their discharge needs. Applies InterQual criteria to determine medical necessity and level of care. Works closely with the physicians and nursing staff to facilitate patient movement to the appropriate level of care. Provide appropriate linkages for patients to community resources. Collaborates with Social Worker colleagues to meet complex social and discharge planning needs. Requires a California RN license and a minimum of 1 year acute care experience.
